A whiteboard, a stack of spreadsheets and SharePoint, replaced with software that fits how they work.

ClearView runs long-term care facilities. We built two internal tools for them: CareBoard, for daily resident follow-up, and Risk Management, for incident reporting and review.

Project one

CareBoard: one place for resident follow-up.

How it worked before

Each facility tracked follow-ups on a whiteboard, backed by a heap of Excel spreadsheets. Staff erased and re-copied items by hand, and nobody had a single view of which residents still had something open.

What we built

A web app that holds the same daily information in structured forms, so an item stays on the board until someone marks it done.

  • Carry-over that doesn't lose things. Items still open at the end of the day roll forward automatically.
  • A Follow-Up view. Every open item grouped by resident, listed once with the date it was first raised, even if it has carried over for days.
  • Access by role and location. Staff see their own facility. Directors and administrators see more.
  • Reports and an audit log. Changes are recorded, and managers can pull history without opening a spreadsheet.
  • Forms admins can change. Admins adjust the form sections themselves when the process changes.

Project two

Risk Management: cases that get finished.

How it worked before

Incident cases were stored and sorted in SharePoint. Nothing required a case to be complete, so some sat half-filled, and the facilities carried that risk themselves.

What we built

A guided incident system where each case moves through a defined review path, and stalled cases get chased instead of forgotten.

  • Step-by-step incident forms. One for each incident type: falls with major injury, resident-to-resident altercations, staff-to-resident allegations, and new or worsened pressure injuries.
  • A review path with owners. Staff submit, regional directors approve, reject, or send it back asking for more information. Each action is recorded.
  • Reminders by email. Cases waiting on a response or a sign-off trigger scheduled reminders to the person responsible.
  • Documents on the case. Supporting files attach to the incident instead of living in a separate folder.
  • Live status. Dashboards update as cases change hands, scoped to each user's facility or region.
  • An admin form builder. The forms can be edited without a developer.

How we approached it

Learn the current system first.

We start by watching how the work really gets done, including the whiteboard, the spreadsheets and the workarounds, and write down where it breaks. Then we build the smallest version that fixes that, put it in front of the people using it, and change it based on what they tell us.

Both ClearView tools have been revised after staff feedback since launch. The aim is software that matches how a facility works, so people use it without being told to.
